At Pro Dental Group in Sacramento, we understand you are very busy and we have made an effort to minimize your time in the office, away from your normal life. That is why we offer some of our forms online, allowing you to complete the necessary patient paperwork in the comfort of your own home on your own schedule. Simply print each form and bring with you to your appointment.
